Rethinking Rehab: Substance Abuse Treatment and the Rise of Mental & Wellness Centers in Islamabad

For a long time, addiction treatment in Pakistan followed a narrow script: a short stay for detox, some general counseling, and a discharge with little real follow-up. That approach is slowly being replaced. A number of mental and wellness centers in Islamabad are now designing rehab programmes around a broader, more accurate understanding of addiction — one where the body, the mind, and long-term stability are treated as parts of the same problem, not separate ones.

The Limits of Detox-Only Treatment

Detox is necessary, but it was never meant to be the whole solution. Managing withdrawal safely gets a substance out of someone's system — it doesn't explain why they started using in the first place, or give them the tools to stay well afterward. Untreated trauma, chronic anxiety, depression, or ongoing family stress tend to remain exactly where they were before treatment began.

This gap explains a pattern many families in Pakistan are familiar with: a loved one completes a program, seems stable for a while, and then relapses within months. The substance was addressed. The underlying condition driving it usually wasn't.

Building a More Complete Rehab Programme

A properly structured rehab programme today tends to include several connected phases, rather than ending once withdrawal symptoms subside:

  • A full intake assessment covering both substance use history and mental health background
  • Medically supervised detox, managed safely and with appropriate clinical oversight
  • Psychiatric involvement, treating co-occurring conditions like depression, anxiety, or trauma alongside the addiction itself
  • Therapy-based rehabilitation, focused on coping skills and behavioral change rather than abstinence alone
  • A structured aftercare plan, including family support and relapse-prevention strategies once treatment ends

This is the model that facilities such as Healing Door Rehab have been working to establish in Islamabad — treating addiction as a condition with psychological roots, not just a physical habit to break.

Where Mental & Wellness Centers Fit In

The shift toward mental and wellness centers reflects a simple but often overlooked reality: a large share of addiction cases involve an underlying psychiatric condition that was never formally diagnosed. Someone using substances to manage untreated anxiety, insomnia, or depression is unlikely to stay sober through detox alone, because the reason they started using hasn't gone anywhere.

Centers built around this understanding integrate psychiatric services directly into the treatment process, rather than treating mental health as a separate concern to be addressed elsewhere, if at all. That distinction — addiction care and psychiatric care under one coordinated plan — tends to produce far more durable results than treating either issue on its own.

A Local Problem Needs a Local Approach

Effective treatment rarely transfers cleanly from one context to another. Cultural attitudes toward addiction, family involvement, access to specialists, and even the substances most commonly seen all vary by region. In Pakistan, methamphetamine — locally known as "ICE" — has become an increasingly difficult case for treatment centers, largely because its effects so closely mirror serious psychiatric symptoms like paranoia and hallucinations.

That overlap is exactly why generic, imported treatment models tend to underperform locally. A programme offering ICE rehabilitation that's built specifically around the realities of substance use in Pakistan — family dynamics, privacy concerns, and local stigma — is generally far better positioned to help patients stay in recovery than a one-size-fits-all approach.

Questions Worth Asking Before Choosing a Programme

Families evaluating treatment options can usually tell a strong programme from a weak one by asking a handful of questions:

  • Is a psychiatrist actively part of the treatment team, rather than just available on referral?
  • Are co-occurring mental health conditions treated alongside the addiction, or ignored?
  • Does support continue after discharge, or does it end there?
  • Is the setting private and stigma-aware, given how sensitive addiction still is in many communities?

These details tend to matter far more than the length of a stay or how comfortable the facility looks on the surface.

The Bigger Picture

The move toward integrated mental and wellness care isn't a trend so much as a correction — a recognition that substance abuse is almost never just about the substance. As more centers across Islamabad adopt this model, combining psychiatric care with structured rehab programmes and genuine aftercare, the odds of lasting recovery for patients and their families continue to improve.
